功能性肠病多困扰,如何利用菌群来治疗?

功能性肠病患者的菌群发生了啥变化?不同微生态治疗手段的效果如何?

功能性肠病,如肠易激综合征(IBS)和功能性便秘(FC)等,正在影响着越来越多的人。数据显示,在我国,IBS 的发病率为 7.5%,FC 的发病率为 6%。然而,针对 IBS 和 FC,目前尚未有良好的治疗手段。不过近年来的研究表明,微生物组或许在这类疾病的发生和发展过程中起着重要的作用。

今天,我们共同关注功能性肠病与微生物组之间的关系。希望本文能够为相关的产业人士和诸位读者带来一些启发和帮助。

     
功能性肠病

肠易激综合征(IBS)和功能性便秘(也叫做慢性特发性便秘,FC)是当前最普遍发生、研究最充分的两种功能性肠病1。虽然这两种疾病并不致命,但它们会影响生活质量、人际关系和生产力,并对个人和整个社会造成了巨大的经济负担。

此前,FC 和便秘型 IBS(IBS-C)被认为是同一种疾病,但现在它们被认为是不同的两种疾病,具体的诊断标准如下:

(1)IBS的诊断标准

按照罗马 IV 标准,当患者在过去 3 个月内平均每周至少有1天出现复发性腹痛,且伴有以下 2 项或 2 项以上异常改变时被诊断为 IBS1

•排便(排便可能会增加或未发生改变)

•大便频率的变化

•大便形态或外观的变化

(2)FC的诊断标准

FC 患者不应符合 IBS 的标准,虽然可能也会出现腹痛和/或腹胀,但并不是主要症状。症状应至少在诊断前 6 个月出现,并在过去 3 个月也应表现出症状。症状必须包括以下两种或多种:

•超过 1/4(25%)的时间排便感到费力

•超过 1/4(25%)的时间排便干硬(参照 Bristol 粪便性状量表-BSFS 分级 1 或 2 级)

•超过 1/4(25%)的时间排便有不尽感

•超过 1/4(25%)的排便时间有肛门直肠梗阻或阻塞感

•超过 1/4(25%)的排便时间需要手法辅助(如用手指协助排便、盆底支持)

•每周自发排便少于 3 次

     
肠道微生物组和IBS

虽然 IBS 的发病机制是多因素的,但肠-脑轴的概念已经作为一个有用的范式,来解释从皮层到肠道肌肉、神经和粘膜的各种功能障碍,以及所导致的不同症状表现。这些表现包括内脏超敏反应、肠道运动改变、中枢高度警觉和应激反应加重。其他因素,如遗传倾向、心理困扰和神经激素等也会产生相互作用,导致 IBS 不同的表型,以及不同严重程度的症状3-6

最近,肠道和大脑之间的连接又增添了一个新的参与者——微生物组,并引出了微生物-肠-脑轴的概念。事实上,动物模型中有诱人的证据表明,肠道细菌可能会影响“大脑”,甚至导致神经和神经精神疾病。因此,微生物组已经成为各种疾病诸如 IBS、帕金森病(PD)和抑郁症的潜在治疗靶点7

有大量的实验数据表明,肠道微生物有能力影响肠道屏障、肠道免疫系统、胃肠道神经肌肉以及中枢神经系统的结构和功能8-11。事实上,将 IBS 患者的粪菌移植到无菌小鼠的实验已经证明,粪菌会改变胃肠道转运并诱发内脏超敏反应12,13

由于在发生急性肠道细菌、病毒或寄生虫感染后可能会出现 IBS 的症状,因此肠道微生物组的变化可能是造成 IBS 的一个因素。最近,测序技术已被用于研究 IBS 患者粪便和结肠微生物组,以及探究各种临床和人口统计数据与微生物组之间的关系。尽管数据仍然有限,而且并不总是一致的,但很明显的一点是:与健康个体相比,IBS 患者的粪便微生物组发生了改变15

由于研究群体数量的有限性,以及患者选择和方法上的差异,目前如何解释可用的数据是一个挑战,更不用说还有未能解释的诸如饮食、治疗、精神疾病共病和症状严重程度等混杂因素的影响。

尽管如此,这些数据还是有一些共同点:IBS 患者的粪便和结肠黏膜中的微生物组是不同的,粪便微生物组不仅可以预测症状严重程度,而且还可以预测其对一种常见干预——低 FODMAP 饮食的反应。

现在非常清楚的是,期望用单一微生物表征 IBS 是非常幼稚的想法。由于 IBS 的症状和严重程度的异质性,以及饮食、性别、药物和其他因素对病情的影响,关于表征 IBS 的微生物组这一问题的答案目前还难以明确,我们应该做好长期准备,以攻坚这些问题。

不过,最近一项纵向多组学研究的数据,能够识别 IBS 亚型特异性,以及与症状相关的微生物的组成和功能变化,也能够将某些细菌代谢物和宿主体内与 IBS 相关的生理机制联系起来18

紊乱的微生物组或宿主对微生物组的异常反应很可能涉及到腔内分子的产生,这些分子对运动、感觉、肠道屏障功能、免疫激活,以及中枢神经系统的交流产生了生物学影响。

小肠细菌也可能发挥作用:尽管小肠细菌引起 IBS 的确切发病率和它在其中发挥的作用尚不明确,但已有证据表明小肠细菌过度生长(SIBO)与 IBS 有关19

SIBO 目前的诊断方法主要是空肠抽吸培养和呼气试验,但两者都具有各自的局限性20。迄今为止最详细的研究证明,IBS 患者体内的小肠微生物组发生了改变,但 SIBO 与 IBS 的临床症状无关21

虽然微生物-肠-脑轴这一概念的出现令人兴奋,但离我们理解它在 IBS 症状发生中的确切作用还有很长的路要走。

与此同时,与调节微生物组的干预措施的临床试验给出了关于肠道细菌在 IBS 中起作用的最令人信服的临床证据,这些干预措施包括饮食、抗生素、益生元、合生剂、益生菌和粪菌移植/转移22,23。但这些干预是如何在治疗 IBS 中起到效果的,目前还不清楚。

     
饮食、微生物组和IBS

膳食纤维是治疗 IBS 历史最悠久的饮食干预措施之一。(合成和天然的)可发酵、可溶性纤维可以增加粪便的频率,改善粪便性状的稠度并加快胃肠道运输,因此成为了治疗以便秘为主要特征的 IBS 患者的最佳选择之一。

纤维是肠道微生物产生乙酸、丙酸和丁酸等短链脂肪酸(SCFAs)的关键底物,而且还可以为结肠细胞提供能量来源,并调节肠道完整性、免疫和通透性24

低 FODMAPs 饮食在 IBS 的治疗中得到了广泛的应用,其疗效得到了临床试验的支持25。FODMAPs 是一种小分子的、具有渗透活性的碳水化合物,在小肠中不易被吸收,但可以被结肠微生物快速发酵并释放出气体和 SCFAs,导致 IBS 症状的恶化,如腹痛和腹胀。因此,毫无疑问,低 FODMAP 饮食会影响结肠微生物组,导致一些重要的共生菌的减少26,然而这一发现的长期后果目前尚不清楚。

     
益生元、益生菌、后生元和IBS中的微生物组

Huaman 和他的同事在一项随机对照试验中,比较了低 FODMAP 饮食和低聚半乳糖(GOS)益生元对功能性胃肠疾病和肠胃胀气患者的作用27。与预期一致,低 FODMAP 饮食会降低双歧杆菌丰度,而益生元则会增加其丰度;两种饮食对沃氏嗜胆菌丰度的影响则与双歧杆菌相反27

除了在益生元组中肠胃胀气和肠鸣音异常两种症状的减轻不显著外,两种策略使其他症状减轻的程度同样显著。事实上,益生元并没有加剧这些“气体相关”的症状,原因是微生物组通过将代谢转向低产气途径,从而适应了 GOS 的摄入28

目前已有多项关于益生菌和 IBS 的研究,积累的数据确实表明益生菌对 IBS 有有利的作用22。然而,由于研究设计、益生菌菌株、剂量和配方以及研究人群和结果的差异,很难选出一种最佳菌株。

实验数据支持益生菌对 IBS 的作用的合理性,因为益生菌对肠-脑轴信号、运动、内脏感觉、肠道屏障、粘膜和全身炎症反应的影响已经在各种动物模型中得到证实29。关于人的有限数据表明,某些益生菌可以影响大脑功能30-32

“细菌治疗”的另一种方法是使用死亡或灭活的细菌、细菌成分或其产品——也就是后生元。这种方法有许多实际和商业上的优势,最近,一种含有热灭活的双歧杆菌 HI-MIMBb75 的制剂被证明能在作者所谓的“现实生活环境”中显著缓解 IBS 及其症状33

     
抗生素和IBS

利福昔明已被证明对非便秘型 IBS 有效,短短 14 天疗程即可诱导一些应答者的长期缓解,同时再次用于治疗也是有效的34。但利福昔明的情况可能比乍看之下更为复杂——它在 IBS 中的作用可能并不依赖于根除 SIBO 或调节细菌种群34

粪菌移植和IBS

基于 IBS 患者肠道微生物组落受到干扰(也就是菌群失调)的假设,以及粪菌移植/转移(FMT)在治疗严重或复发性艰难梭菌感染方面有着成功且总体良好的安全记录, FMT 也被用于治疗 IBS36-46,尤其是在 IBS 缺乏有效治疗手段的条件下。

迄今为止的研究结果有些混杂,一些研究表明了 FMT 的益处36,38,40,42,43,而有些研究则没有37,39,41。这些研究成功的原因各不相同,包括剂量、输送方法、受赠人群的性质和捐赠材料的组成,但具体的原因还有待确定。FMT 有利的反应似乎与受赠人微生物组和 SCFA 生成的某些变化有关。

功能性便秘

与 IBS 相比,关于 FC 患者微生物组组成的研究很少47-50;一个清晰的模式尚未被发现。除了上述与 IBS 相关的多种混杂因素外,对 FC 患者微生物组的研究还必须考虑另外一个因素的影响——结肠转运缓慢和由此导致的粪便淤积47,49

是微生物组的改变改变了结肠的运动,还是延迟的结肠转运改变了结肠微生物组的组成?

无论便秘的病因是什么,纤维都是一种历史悠久且有效的治疗便秘的方法51,几十年来人们一直知道纤维可以改变结肠中细菌的密度52,53。纤维和益生元对细菌的增殖作用相对于其他作用的优势仍有待确定。

同样,与 IBS 相比,关于调节 FC 患者结肠微生物组的其他干预措施的研究相对较少。益生菌或 FMT 可以通过多种微生物相关机制(例如通过 SCFA 的产生或胆盐代谢)改善便秘相关的症状54,55。然而,目前关于益生菌对 FC 的临床疗效的证据还很薄弱,需要进一步的研究56,57

     
结论

鉴于微生物组研究技术的进步和从动物模型中得到的令人振奋的数据,微生物-肠-脑轴被认为与 IBS 相关。对许多人来说,它已成为我们理解 IBS 的核心模型——肠-脑轴的逻辑延伸。“紊乱的微生物组或宿主对微生物组的异常反应,可能与 IBS 相关,并会影响中枢神经系统”这一猜测的可能性目前正在被认真考虑和研究,并有可能为这种具有挑战性的疾病打开新的诊疗前景。

由于许多现有数据都来自动物模型,在解释它们时必须保持谨慎——没有一个单一的动物模型可以概括 IBS 的全部表型。还必须谨记微生物-肠-脑轴相互作用的双向性——炎症和肠道微生物组之间复杂的相互作用说明了疾病状态是如何影响微生物组的。现在有许多有趣的干预措施,但似乎只有一些方法可以针对 IBS 患者进行个性化药物治疗。
